How To Get Started—Initial Evaluation
It’s always best to request an evaluation, especially when you’re not sure if your child even needs services. We use a simple four-step process to help you get started.

Call Our Office
Before calling our office, please have a copy of your insurance card available to share with a member of our staff. When you call, we will ask a few questions about your child to determine if we have the services your child needs.

Get a Referral
Call your primary care physician’s office and request that they send a referral to RISE Pediatric Therapies. Our fax number is (208) 467-1462.

Complete Intake Paperwork
Once we receive the referral, we’ll send you a link to our electronic medical record system to complete your intake paperwork. After that’s done, we’ll schedule your evaluation. The intake packet must be completed by 6:00 p.m. the day before your appointment. If it isn’t, we’ll need to reschedule.
If you have any questions or trouble completing the packet, please call our office as soon as possible.

Come to Your First Appointment
Please arrive 10–15 minutes before your scheduled appointment to ensure all paperwork is completed and all documents are signed and dated.
Once the initial evaluation is completed, we’ll develop a coordinated care plan for therapy.
Feeding Therapy for Easier Mealtimes
Sometimes children need support with the processes of feeding and/or swallowing. Our pediatric feeding therapy, offered at our Nampa location, can make a difference. We work closely with your child to develop their feeding skills while ensuring you feel confident supporting their progress at home. Together, we’ll make mealtimes more manageable and less stressful for your family.
Myofunctional Therapy for Better Muscle Control
Issues like tongue thrust, tongue tie, or TMJ can affect how your child speaks, eats, or even breathes. Pediatric myofunctional therapy helps retrain the muscles in the face, throat, and mouth. Over time, this therapy can significantly improve the functions that depend on those muscles, making daily activities easier for your child. We provide this therapy at our Nampa clinic.
Speech & Language Therapy for Communication Skills
If your child needs support in meeting the developmental milestones for speech or language, we are here to help. We offer speech and language therapy at our Boise clinic to improve their speaking, listening, and comprehension skills.
