How To Get Started—Initial Evaluation
Even if you’re not sure if your child needs services, it’s always best to request an evaluation. We use a simple four-step process to help you get started.

Call Our Office
Before calling our office, please have a copy of your insurance card available to share with a member of our staff. When you call, we will ask a few questions about your child to determine if we have the services your child needs.

Get a Referral
Call your primary care physician’s office and request that they send a referral to RISE Pediatric Therapies. Our fax number is (208) 467-1462.

Complete Intake Paperwork
Once we receive the referral, we’ll send you a link to our electronic medical record system to complete your intake paperwork. After that’s done, we’ll schedule your evaluation. The intake packet must be completed by 6:00 p.m. the day before your appointment. If it isn’t, we’ll need to reschedule.

Come to Your First Appointment
Please arrive 10–15 minutes before your scheduled appointment to ensure all paperwork is completed and all documents are signed and dated.
Once the initial evaluation is completed, we’ll develop a coordinated care plan for therapy.
Feeding Therapy
Some children may present with eating or swallowing difficulties. Our pediatric feeding therapy offered in Nampa for Meridian families helps children to develop the skills they need for easier and safer mealtimes. We also guide parents on how to continue the progress at home, providing confidence and tools for success.
Myofunctional Therapy
Issues like tongue tie, tongue thrust, or TMJ can impact muscle control in the face, mouth, and throat.
